2.初診於本治療所，接受治療之患者，依照「 物理治療師法之物理治療所設置標準 」之法規，請攜帶 「 醫師開立之診斷 , 照會或醫囑 」。
3.如有相關 「 醫學影像檢查 」資料或光碟（ 例如 ：X-RAY / CT / MRI ），請攜帶至本治療所，以便佐證之評估 。
4.本治療所一律採 「 預約制治療 」，若您臨時不克前來或其他個人因素，請務必「 來電 」更改治療時段，以避免佔用他人醫療權利，與喪失日後優先預約資格 。
